APPLICANT STATEMENT

I am Muslim, I believe that there is no deity except Allah (The God Almighty) and Muhammad (pbuh) is His Messenger. I agree to abide by the decision of the Board of Directors as the disposition of this Application. If admitted to membership, I agree to be governed by the By-Laws of the Islamic Center of Northeast Florida.

4. Enclose payment for the first month fee
5. Send it to the Islamic Center at the following address

Please enclose at least first month payment to Islamic Center on the following address or contact Treasurer after filling out the automatic deduction from your checking account or credit card.
